What is the no-incision facelift?

With a no-incision facelift, we can use laser liposuction to remove fat from the jaw and neck areas and then use a sophisticated fat grafting technique with the patient’s own fat to restore fullness to the upper face. No incisions are necessary. We only need to make small punctures hidden under the chin and in the normal creases of the face.

The wonderful thing about this advanced approach is that it can be done in the office in one simple visit under local anesthesia and mild sedation. There’s really no recuperation aside from one week of minding how you sleep, what you eat, and how much you talk. 


Should I schedule this procedure?

As a younger man in training to be a plastic surgeon, Dr. Lappert was taught how to do a facelift. If a patient came into the office complaining that they looked too old, a facelift was essentially the only treatment that could be offered.

The only questions to be asked were, “Are we lifting the brow as well, do we need to treat the eyelids with an ‘eye tuck’ (blepharoplasty) and are we going to add some treatment to make the skin look younger (chemical peel, laser)?” The basic treatment was still the facelift.

In the early days of plastic surgery, we thought people looked old because they had too much loose skin. This meant that the early facelift was about tightening the skin. Over time, plastic surgeons came to understand that the more important issue was what happened underneath the skin to make the face look old.

We saw that the soft tissue under the skin appeared to have slipped down the face as if gravity had pulled everything downward. In the 1970s, plastic surgeons started lifting up the soft tissues under the skin to make the face look younger. Over time, the technique was modified in ways that surgeons thought would provide a more durable result.

These operations are still very popular, but recently there has been a huge revolution in how we can make faces look younger. To understand it, we first have to understand why faces look older.

The pictures above show Dr. Lappert when he was 24 years old, and at age 59. These pictures show the main effects of aging. As we age, some of the fat cells in our faces shut down and virtually disappear, while others can awaken and become chubby. Fat cells in the upper half of the face can disappear. This makes the muscles of our lower eyelids visible (the “bags under the eye”). At the same time, the fat cells in the lower face get bigger, obscuring our jaw, and giving us the “jowls.” The young face is fuller at the top and narrower at the bottom. The old face looks rounder because the top got smaller and the bottom got bigger.

We call this the deflation wave. Just like “the wave” at a football game, when people stand and sit as the wave goes around the stadium. In this case, the fat cells at the top of our faces have permanently sat down and the ones around our jaws and necks have stood up.

Notice how none of the above information discusses the concept of “too much skin.” It’s all about fat in the wrong places.

What should I expect from the results?

Look at the three pictures below. The first is before, the second is 6 months after, and the third is 5 years post procedure. That is a lasting result!
